You can get W-2 forms from the local office supply store, from your CPA or download them from the IRS. W-2 forms are also available from most tax preparation software.

W-2 forms, along with the W-3 form, should be submitted to SSA by January. 31 of the next year. Jan. 31 is the deadline for employees to get W-2 forms. The form W-3 sends data to the SSA. It combines all the information on W-2 Forms into a single total.
